The purpose of ovarian stimulation is to stimulate as many ovaries as possible and ensure the formation of more than one egg. The reason for this is that the best embryos to form are selected and placed in the uterus.Ovarian stimulation involves suppressing the normal cycle with GnRH analogues and stimulating the ovaries with stimulants. Meanwhile, the size of the ova is regularly monitored by blood hormones and ultrasound. Thanks to this follow-up, overstimulation of the ova is prevented and the egg retrieval time is adjusted.
Collection of ovules
After stimulation of the ovaries, the follicles that reach a sufficient size on ultrasound are collected using vaginal ultrasound with a thin needle. An injection is given before this procedure to prevent the ova from bursting, and the ovum retrieval procedure is performed 34-36 hours after the application. During the procedure, the patient is put to sleep with sedation or general anaesthesia. After the ovum collection, which takes about 15-20 minutes, the patient rests for an hour and is sent home.
Microinjection
Microinjection is used when sperm count and quality are insufficient with conventional in vitro fertilisation or in cases where fertilisation cannot be achieved with classical in vitro fertilisation. The sperm cell is injected into the ovum using microsurgical methods. The difference between microinjection and classical in vitro fertilisation is the fertilisation phase and is preferred when sperm are problematic. The process is to melt the cells around the ovum and prepare the ovum for microinjection. The maturity of the ovum is then assessed and the sperm is applied to the ovum by microinjection.
Embryo transfer
In IVF treatment, a certain number of embryos are placed in the uterus after fertilisation. This procedure is called embryo transfer. Embryo transfer takes place 3 days after egg collection. However, in special cases, embryo transfer (blastocyst) is carried out 5 days later. When the number of embryos to be transferred is high, the likelihood of pregnancy and the rate of multiple pregnancies increase. Multiple pregnancies may result in premature birth. The aim is to increase the chance of pregnancy without increasing the risk of multiple pregnancy.With the recommendation of the Ministry of Health in our country, the number of embryos to be transferred should not exceed 3. However, if the particular situation of the patient is questioned, the number of embryos to be transferred can be increased. In embryo transfer, the age of the patient, whether in vitro fertilisation has already been performed and the quality of the embryo are important. Embryo transfer is a procedure that takes up to 5 minutes without the need for anaesthesia. Before the transfer, the patient is prepared in the appropriate surgical environment. A special speculum is placed in the vagina and cleaned with gauze, the cervix is washed with special culture fluid and the embryos brought from the laboratory are transferred. It is usually a painless procedure, very rarely slight cramps are felt. The patient is sent home after at least half an hour of rest.
